In testimony of my devotion to my Lord and in acceptance of my responsibility as a member of Siloam Presbyterian Church, I have indicated below the types of service I am willing to perform.
How to use the form

- For each item, mark “Willing to serve” if you would take it on, and “Have experience” if you have done it before. Both can be true.
